It turns up by itself when you join, no forms.

Your AI number arrives on its own when you set up the app. You'll see it on the last set-up screen and under Your numbers, where a tap copies it.

Can I choose it? No, sorry. Numbers are given out from the ones available, so there's no picking digits or area codes. Your simp will make it look good anyway.

Can I swap it later? No, your AI number stays the same.
